Behind the scenes at the UK's most remote hospital. At the Gilbert Bain in Shetland the team of doctors and nurses work alongside GPs, paramedics, coastguard and the fire & rescue service to provide emergency and medical care to the islands' 23,000 residents. They also deal with oil rig workers, fishermen and tourists 24 hour a day, 365 days a year. Shetland throws up unique challenges for this tight-knit team. Today, patients from Britain's most remote community - on Fair Isle, south of Shetland - wait for a weather window to make the trip for important operations. They include John, who is hoping to get the all-clear after surgery to find out whether cancer has been kept at bay. GP Esther Mitchell makes the trip the other way to see patients on Fair Isle. Poor weather had meant it has been eight weeks since she was able to keep her appointment. And a specialist surgeon travels to Shetland to carry out a complex operation on a Fair Isle resident, who is keeping her fingers crossed that fog lifts to get her home again afterwards.

More action from the UK's most remote hospital. At the Gilbert Bain in Shetland the team of doctors and nurses work alongside GPs, paramedics, coastguard and the fire & rescue service to provide emergency and medical care to the island's 23,000 residents. They also deal with oil rig workers, fishermen and tourists 24 hour a day, 365 days a year. For most hospitals in the UK, the winter months are often the busiest. On Shetland, summer is when the pressure's on. The A&E staff have to juggle the available space when a patient arrives with mystery abdominal pains. She's in a lot of discomfort but it isn't immediately obvious what the problem is. Dental Director Dr Brian Chittick operates on one of his own colleagues who has two troublesome wisdom teeth that need to be removed in surgery. And the stars of the women's rugby sevens' team - including Shetland's island medics - take on teams across Scotland in the summer competition.
