With flames emerging through the cockpit floor, the pilots of Pilgrim Airlines 458 are desperate to land. Amid smoke, extreme heat and zero visibility, they somehow manage to touch down on a frozen Rhode Island reservoir. Within minutes, the inferno destroys any evidence of how the fire started.
